fee is $200.00 for a 45 minute session, $50.00
for group session.
Upon
request, I
will
provide
a
bill
for
paid
services
which
you
may
submit
to
your
insurance
company
for
reimbursement. I am accepted as an out-of-network provider for most insurance carriers.
Since
coverage
varies
widely,
it
is
recommended
that
you
contact
your
insurance
company
to
determine
which
services
your
policy
includes.
